How Spravato compares to other depression treatments

Spravato (esketamine)

Spravato and ketamine are not usually first-line treatments for depression. There are some logistical considerations for this in-office treatment but the potential benefits make it an important option.

Transcranial Magnetic Stimulation (TMS)

TMS involves using non-invasive magnetic pulses to stimulate areas of your brain that depression has altered or shut down. It’s usually only used after more traditional depression treatments have failed. TMS does not require medication and patients are completely awake during the painless, outpatient treatment.

Antidepressants

Adjunctive Treatments

This is when a doctor prescribes additional medications to try and boost the effectiveness of your antidepressants. The most common are mood stabilizers, anti-anxiety medications, and atypicals (SGAs).

Second Generation Antipsychotics (SGA)

SGAs can be administered on their own or as an adjunctive antidepressant treatment. Their name is confusing as they are frequently prescribed for non-psychotic symptoms.

Ketamine vs. Spravato®: What’s the Difference?

Spravato® (esketamine) and ketamine are related medications that work through similar neurobiological pathways involved in mood regulation and neuroplasticity.

Spravato® is an FDA-approved nasal spray specifically indicated for treatment-resistant depression and certain patients with major depressive disorder and acute suicidal thoughts or behaviors. Ketamine has been used safely in medicine for decades and may be prescribed off-label for depression and other psychiatric conditions.

While the treatments are related, they differ in formulation, FDA approval status, administration methods, treatment protocols, and insurance coverage. Both may be effective options for patients who have not responded adequately to traditional antidepressant treatments.

Do you offer both Spravato® and ketamine-based treatments?

Yes. Wave Treatment Centers offers both Spravato® and ketamine-based treatments as part of our interventional psychiatry program.

Because every patient has a unique clinical history and treatment response, we perform a comprehensive evaluation to determine which treatment option may be most appropriate based on symptoms, prior treatments, medical considerations, and treatment goals.

How do I know whether Spravato® or ketamine is right for me?

There is no single treatment that is best for everyone. Factors such as diagnosis, previous medication response, insurance coverage, treatment history, medical conditions, and personal preferences all play a role in treatment selection.

What is Spravato®?

Spravato® (esketamine) is an FDA-approved nasal spray medication used to treat treatment-resistant depression and certain patients with major depressive disorder and acute suicidal thoughts or behaviors. It is administered under medical supervision in a certified clinical setting.

How does Spravato® work?

Spravato® works differently from traditional antidepressants. It targets glutamate pathways in the brain, which may lead to faster symptom improvement in some patients.

What should I expect during a Spravato® treatment?

Spravato® is administered in a certified medical setting under clinical supervision.

After self-administering the medication under staff guidance, patients remain in the clinic for approximately two hours of observation and monitoring. During this time, they are seated in a comfortable setting and may rest, listen to music, read, or simply relax while the effects of the medication gradually take effect and then wear off.

Common experiences during treatment may include relaxation, mild dissociation, or perceptual changes, which are temporary and monitored closely by clinical staff.

Because effects may persist after the monitoring period, patients may not drive themselves home following treatment and must arrange transportation.

How fast does Spravato® work?

Some patients notice improvement within hours to days, while others may require several weeks of treatment sessions before experiencing significant benefit.

How long does Spravato® treatment last?

Spravato® is typically administered twice weekly during the initial phase, with frequency gradually reduced over time based on clinical response.

Is Spravato® covered by insurance?

Is ketamine covered by insurance?
